The short answer
Most kitchens do not lose a rating to one disaster. They lose it to a long tail of small, fixable faults across the three scored areas - each minor on its own, but enough together to pull the total down a band.
Under the Food Hygiene Rating Scheme an officer scores food handling, structure and cleanliness, and confidence in management, then adds them. Because a high score in any one area drags the whole rating down, a scatter of small problems in the same area does real damage. Here are the quiet ones, grouped by the area they hit.

That is well over two dozen quiet drags, and most are preventable. A deep clean clears the structural list in one pass and supplies dated evidence for the records; the handling and management lines are yours to keep in daily order. Together they are the difference between a 3 and a 5.

Because the officer adds them up. The three areas are scored in points, and a cluster of small faults in one area lifts that area's score - and a high score in any single area drags the whole rating down a band. Several minor issues in the same area can cost you a whole rating.
The structural and cleanliness list - grease, hidden soil, extraction, high-level dust - is the fastest to clear, because a single planned deep clean reaches all of it. The handling and management lines need daily discipline and good records rather than a one-off fix.
It can be. An officer can ask to see your TR19 grease cleaning certificate as evidence the system is maintained. An absent or out-of-date one is noted in the report and may become a formal requirement to address.
Yes, sometimes more. Confidence in management is a full scored area. A missing or unused food safety management system can cap your rating even if the kitchen is visibly clean, so records and training matter as much as surfaces.
