The short answer

On a long run, reaching the middle is the whole problem

A long horizontal duct collects debris along its entire length, but the part that matters most is the part furthest from any opening. Cleaning the ends of a run is easy; cleaning the middle - the stretch you cannot reach from either end - is where a run gets skipped. On long horizontal ductwork the challenge is not the cleaning technique but the access, and access is what decides whether the whole length actually gets clean.

Why long horizontal runs collect debris

Low velocity lets it settle

Air slows as it travels along a duct, and where it slows, whatever it is carrying settles out. On a long horizontal run there is plenty of length for dust, lint or grease to drop onto the bottom of the duct, and the flatter and longer the run, the more it accumulates. Bends and junctions add local slow spots that collect more. The result is a layer of debris spread along the whole run, thickest in the low-velocity stretches - often right in the middle, exactly where access is worst.

That debris is the same problem it is anywhere: restricted airflow, and in a grease or lint run, a fire load. The difference on a long horizontal duct is simply that it is spread over a distance that is hard to reach in one go.

Why access is the real constraint

You can only clean what you can reach

A cleaning method can only work on the length it can physically reach. From an open end or a single panel, tools and cameras reach so far and no further, so a long run needs access points spaced along it - which is why guidance calls for inspection and access panels at regular intervals, commonly around every couple of metres, so every section can be reached, cleaned and verified. Where a run was installed without enough access, whole stretches in the middle can be impossible to clean or even inspect properly, and they are usually the dirtiest.

Adding access where it is missing is often part of doing the job properly. Without it, a clean can look complete at the accessible ends while the worst of the debris sits untouched in the middle - the section no one could get to.

How a long run gets cleaned properly

Survey, access, verify the whole length

Doing a long run properly starts with a survey that finds where the debris is and where the access is - or is not. From there the run is cleaned section by section through adequate access points, with any missing access added where needed, and verified along its whole length rather than just at the reachable ends. Inspection cameras confirm the middle is actually clean, not assumed clean. It is more planning than a short run needs, but it is the only way to be sure the stretch that was hardest to reach is the one that got done.
