The laundry is one of the busiest and least glamorous rooms in any care home - and one of the most dangerous. Get the heat, the airflow and the extraction wrong, and it quietly becomes a source of both infection and fire.
On the ground
It is half past six in the morning and the laundry is already the warmest room in the building.
Baskets of soiled bedding are stacked by the door. A washer is thumping through its final spin, two tumble dryers are running flat out, and the air carries that unmistakable warm-cotton smell that anyone who has worked in a care home knows well. It looks like the most ordinary room imaginable - and that is exactly the problem. Behind the routine sit two serious hazards that rarely get the attention they deserve: the risk of spreading infection through poorly handled linen, and the risk of fire through heat, lint and residue building up where nobody is looking.
A laundry is not a back-of-house afterthought. It is a piece of clinical infrastructure, and it deserves to be treated with the same seriousness as a kitchen or a medicines room. Get the airflow, the temperatures and the extraction right and it protects residents day after day. Get them wrong and it can put frail, vulnerable people directly in harm's way.
Every sheet, towel and item of clothing that passes through the laundry has come off a resident, and in a care setting that means a share of it will be contaminated with blood, faeces, vomit or other body fluids. The whole purpose of the room is to break the chain of infection - to take soiled linen in at one end and send genuinely clean, disinfected linen out at the other, without ever letting the two meet in between. That sounds simple. In a busy home under staffing pressure, it is anything but.
Detergent alone does not disinfect. The recognised benchmark in the UK is thermal disinfection, set out in the Department of Health and Social Care's Health Technical Memorandum 01-04 for the decontamination of linen. The principle is straightforward: the wash has to reach and hold a temperature high enough to destroy the organisms living in the fabric. The widely cited figures are a load held at a minimum of 71°C for three minutes, or 65°C for ten minutes, with additional time added on top so that heat can penetrate right through to the centre of a packed load rather than just warming the outside of it.
That has real design consequences. A domestic-style machine bought to save money often cannot guarantee those temperatures under load, and a "40° eco" cycle - however kind to the energy bill - is not disinfecting anything. Where residents are known to carry specific infections, soiled and infectious linen is meant to be bagged at source in a water-soluble inner bag and washed separately, without hand-sorting, so staff are never picking through contaminated items. The temperature you can prove matters far more than the one on the dial.
The second half of the infection story is flow. A well-run laundry works in one direction only: dirty linen enters, is loaded, is washed and disinfected, and clean linen leaves through a different route - it never travels back across the dirty zone. The Care Quality Commission looks closely at exactly this when it inspects, asking whether a home genuinely separates clean and dirty, whether there is hand-washing at the point of need, and whether soiled and clean stock are stored apart. Ventilation is part of that separation too: air should move from the clean side towards the dirty side and out, not the other way around, so that airborne contamination is carried away from clean stock rather than settling on it.
If infection is the risk you can reason your way to, fire is the one that catches homes off guard. A care home laundry combines everything a fire needs - continuous high heat, a steady supply of fine combustible dust, and fabrics that can be far more flammable than they look. And unlike a kitchen, where the danger is obvious, a laundry fire builds up silently over months in the places nobody thinks to check.
Every drying cycle sheds lint. The filter catches the visible portion, but a significant amount is carried past it into the machine's internals and along the exhaust ducting to the outside wall. Lint is essentially dry, finely divided fabric - it ignites easily and burns fast. Fire services across the UK are consistent that the most common cause of tumble dryer fires is lint build-up in the dryer and its exhaust duct. A partially blocked duct also traps heat, so the machine runs hotter than it should, drying takes longer, and you are feeding more energy into a duct that is already lined with fuel. Emptying the lint filter after every cycle is the bare minimum; it does nothing for the run of ducting behind the machine, which needs cleaning through on a planned schedule.
Here is the risk that turns an ordinary laundry into something far more dangerous, and it is specific to care settings. Emollient creams - the moisturisers used constantly to manage older and fragile skin - soak into clothing and bedding. The MHRA, working with the National Fire Chiefs Council, has warned repeatedly that fabric with dried-on emollient catches fire more readily and burns hotter and faster than clean fabric, and that this applies to both paraffin-based and paraffin-free products regardless of concentration. Crucially, washing does not remove the risk - the residue builds up in the fibres over time. Between 2015 and 2022 there were 78 confirmed fire deaths in Great Britain linked to emollients. Load that same emollient-laden fabric into a hot dryer and the exothermic risk is real, which is why linen from residents using these creams needs changing and laundering regularly and cooling fully before it is stacked. None of this is exotic. It is heat, airflow and housekeeping - the same three things that decide whether any high-temperature room is safe or dangerous. The laundry earns its keep quietly, and it can fail quietly too. The homes that treat it as critical infrastructure, with the temperatures logged, the flow disciplined and the extraction cleaned on a schedule, are the ones where that warm early-morning room stays exactly what it should be: ordinary.

A low-temperature or eco cycle will clean but will not disinfect, so the machine has to be able to prove it reaches and holds those temperatures under a full load.
Two things combine. First, tumble dryers shed lint that builds up inside the machine and along the exhaust ducting, and lint is dry, combustible and easily ignited - it is the most common cause of dryer fires. Second, emollient creams used on residents' skin soak into clothing and bedding and make fabric burn faster and hotter, and washing does not remove that residue. Together they mean a care home laundry needs its extraction cleaned on a schedule and its emollient-contaminated linen changed and cooled with real care.
