How a Care Home Failed Its Fire Inspection

A care home can tick every box on alarms and drills and still fail its fire inspection on one overlooked system - the kitchen extract. Here is exactly how it happened, and how to make sure it never happens to you.

On paper, the home looked ready. Alarms tested, drills logged, fire doors surveyed, extinguishers in date - and yet the visiting fire safety officer left having served an enforcement notice.

The setting is a mid-sized residential care home with around forty beds and a busy kitchen that runs breakfast, lunch and an evening meal every day of the week. The responsible person - in this case the registered manager - had done what most conscientious managers do. They kept a tidy fire logbook, they had a current fire risk assessment, and they could produce records for the alarm panel, the emergency lighting and the annual door inspections. Staff knew their evacuation roles. By any reasonable measure, this was not a home that took fire safety lightly.

So the enforcement notice came as a shock. It did not turn on the alarm system or the escape routes at all. It turned on a single system almost nobody looks at until something goes wrong - the kitchen extraction ductwork running from the canopy up through the roof void. The officer opened one inspection hatch, ran a finger along the inside of the duct, and the conversation changed completely. What follows is what went wrong, why it counted as a failure in law, and the practical fix that would have prevented it.

What the inspector actually found

The canopy and the visible filters looked clean, because those are the parts the kitchen team wipes down every night. Behind the filters, though, the horizontal and vertical runs of ducting told a different story. Years of frying, grilling and roasting had left a thick, sticky layer of grease coating the internal walls of the extract - the part no cloth ever reaches. In places it had begun to carbonise into a hard, varnish-like crust.

Grease inside a hot extract system is fuel. Around 70% of fires in commercial kitchens are linked to the extract ventilation and the fat and grease that collect inside it, and once a flame reaches a grease-lined duct the fire can travel the full length of the system in seconds, straight into the roof void above sleeping residents. In a care home, where many people cannot self-evacuate quickly, that is close to a worst-case scenario.

Two documentary failures sealed it. First, there was no post-clean verification report to prove the internal ductwork had ever been cleaned to standard. Second, long sections of the duct had no access panels at all, which meant nobody could have inspected or cleaned the interior even if they had wanted to. To the officer, that combination showed the fire risk assessment had never genuinely considered the extract system - a clear gap under the law.

What went wrong

None of this happened through carelessness. It happened because the extract system fell into a blind spot between the people who look after the building and the people who look after fire safety. Unpicking it, four things had quietly gone wrong.

  • The wrong surface was being cleaned. Nightly wiping of the canopy and swapping the baffle filters kept the visible kitchen spotless, but no filter is ever fully effective and grease always carries past it into the ducting. The team was cleaning the 10% you can see and ignoring the 90% you cannot.
  • The extract was missing from the fire risk assessment. Under the Regulatory Reform (Fire Safety) Order 2005, the responsible person must assess and manage every significant fire risk. The assessment covered alarms, doors and escape routes in detail but treated the ductwork as a plumbing detail rather than a fire risk - so nobody had ever set a cleaning frequency for it.
  • There were no access panels. Good practice, set out in the ductwork standards, is to fit inspection hatches at regular intervals along the whole run. Without them, the interior cannot be inspected, cannot be cleaned and cannot be verified. A duct you cannot open is a duct you cannot prove is safe.
  • There was no evidence. Even if occasional cleaning had happened, there was no post-clean verification report with before-and-after readings, photographs and a system schematic. In the eyes of a fire officer - and, just as importantly, an insurer after a fire - if it is not documented to standard, it did not happen.

The recognised benchmark here is TR19® Grease, the BESA specification for managing fire risk in kitchen extraction. It is the standard fire officers, insurers and risk assessors point to, and it is unambiguous: grease must be controlled so the internal surfaces do not exceed a mean average of around 200 microns between cleans, heavier spots of 500 microns or more need dealing with promptly, and a proper clean must bring the system back below an average of 50 microns, proven by a deposit or wet film thickness test. The home could not demonstrate a single one of those numbers.

The fix

The reassuring part of this story is that the remedy is well defined and entirely achievable. The home lifted the enforcement notice within weeks by working through a clear sequence, and it is the same sequence any care home can follow before an inspector ever knocks.

  1. Get a baseline survey. Have the full extract system assessed against TR19® Grease, with thickness readings taken along the canopy, the horizontal runs, the riser and the fan. This tells you the true condition of the parts you cannot see and gives you a documented starting point.
  2. Fit access panels throughout. Where the ductwork cannot be opened, have compliant inspection hatches installed at the intervals the standard calls for, including at every bend and change of direction. This single step turns an uninspectable system into a maintainable one.
  3. Deep clean the whole system. Bring the entire internal run - canopy, filters, ducting, fan and roof-level components - back below the 50 micron average, not just the visible areas. This is specialist work carried out by technicians who hold the BESA Grease Hygiene Technician qualification.
  4. Insist on a post-clean verification report. The clean is only worth what you can prove. A compliant report gives you before-and-after readings, dated photographs, a labelled schematic and the technician’s sign-off. Keep it in the fire logbook next to your alarm and door records.
  5. Set a cleaning frequency and diarise it. A kitchen cooking twelve hours or more a day sits in the heavy-use band and typically needs the extract cleaned every three months; six to twelve hours points to roughly every six months; lighter use every twelve. Match the interval to your kitchen, then book it in as a standing appointment.
  6. Fold it into the fire risk assessment. Finally, write the extract system into the assessment as a named fire risk with its own control measure and review date, so it can never slip into the blind spot again.
