The first two cause the most illness and the most lost ratings, but all four are assessed, and a kitchen needs an answer to each.
The failures that cost ratings are usually ordinary habits rather than dramatic lapses. Raw meat stored above ready-to-eat food, so blood or juices can drip down, is one of the most common and most heavily penalised. Shared chopping boards and knives used across raw and ready-to-eat without a proper clean between is another, which is why colour-coded boards exist - and why they only work if the colours are actually respected. A single cloth wiping a raw surface and then a clean one spreads bacteria straight across the kitchen. Weak hand hygiene between tasks does the same thing more quietly. Storing chemicals next to or above food, or decanting them into unlabelled containers, invites chemical contamination. And a surface that only ever gets a quick wipe leaves a residue that recontaminates the next thing to touch it - the heart of the difference between a kitchen that looks clean and one that scores clean.
Timing and workflow cause the failures that habit hides. Prepping raw meat and salad on the same bench at the same time, even with the best intentions, puts them one splash apart. Defrosting raw food on a shelf above ready-to-eat items lets it drip through the whole thaw. Handling money or a phone and then food without washing hands carries contamination in from outside the process entirely. Reusing a marinade that held raw meat as a sauce, or garnishing a cooked dish with a utensil that just touched raw, undoes the cooking that was meant to make the food safe. None of these looks dramatic in the moment, which is exactly why they persist until an inspector, or a customer's illness, brings them to light.
Controlling cross-contamination is about physical separation and reliable cleaning, not vigilance alone. Store raw below ready-to-eat, always, so nothing can drip the wrong way. Use dedicated or genuinely cleaned equipment for raw and ready-to-eat, and respect the colour-coding rather than grabbing the nearest board. Wash hands properly between tasks, and use clean cloths or disposables rather than one cloth for everything. Keep chemicals stored away from food and clearly labelled. And treat deep cleaning as part of contamination control, because the residue that builds up in the gaps behind and under equipment is both a bacterial reservoir and the kind of hidden harbourage where pest risk builds up behind clean-looking equipment.
